Let us know asap if
something happens
Timing matters when it comes to incidents
Accidents can happen, however, the important thing is that you’re quick to report them and we’re quick to respond.
An incident is when you or someone else gets hurt or property gets damaged, or there’s potential for this to happen. Please REPORT INCIDENTS urgently, so we can take action. Simply CALL or EMAIL Ello Care or use or use THIS FORM.
WHAT HAPPENS NEXT?
If you report an incident, we contact you within 24 hours.
SERIOUS INCIDENTS
The NDIS QUALITY AND SAFEGUARDS COMMISSION must be informed when a serious “reportable” incident happens (or is alleged to have happened), in connection with our disability services or with the participant. Serious incidents can be identified and reported by anyone, for example an Ello Care team member, a participant, a participant’s nominee, family or friends. A reportable incident can include:
Death
Serious Injury
Abuse or neglect
Unlawful sexual or physical contact with, or assault
Sexual misconduct, committed against, or in the presence of, a person with a disability, including grooming of a participant
Use of restrictive practice in relation to a participant, where the use is not in accordance with an authorisation (however described) of a state or territory in relation to the person, or if it is used according to that authorisation but not in accordance with a behaviour support plan for the participant
